i'm trying to design a front end for a database and have no idea what im doing

i have a form which is opened up several times and info needs to be stored somehow each time a new form begins, a guy i know seems to think creating some sort of array for each of the fields contained within the form will work however i have no idea where to start

the feilds contain both numerical and text content.

hope some one can help my boss has got me doing thins as i finnished my original task early

please help
